Good Faith Estimate

Under the No Surprises Act (effective January 1, 2022), health care providers are required to give patients who don't have insurance or who are not using insurance an estimate of expected costs before scheduled services.

"You have the right to receive a 'Good Faith Estimate' explaining how much your medical care will cost. Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services."

Who this applies to

This right applies to uninsured patients and patients who choose not to use their insurance (self-pay clients).

What is included in a Good Faith Estimate

A written estimate of expected charges for services based on your diagnosis and planned treatment. The estimate includes the expected cost for the items and services most likely needed.

How to request one

Dispute resolution

If your final bill exceeds your Good Faith Estimate by $400 or more, you may be able to dispute the bill. For information about the dispute resolution process, visit cms.gov/nosurprises.

SHARE's commitment

We are committed to financial transparency. We will provide a Good Faith Estimate to any eligible patient who requests one, prior to the first scheduled appointment.
